Tipping at restaurants...

Being an Australian, no matter how hard I try I just can%26#39;t get my head around tipping! I have read a little bit about it, 15-20% for meals but what I don%26#39;t know is how are you meant to give it. Just give it as a seperate payment to your waitress before you leave? Or is it added onto the bill? What if you pay on credit? How do you do it then? So confusing! Any help would be appreciated, I dont want to look completely stupid when I come over!

Even when paying on a credit card, we leave the tip in cash on the table, apparently in the US it is not guaranteed that the staff will get the tip if you add it on the card (unlike most restaurants in Australia who are careful about this).

Also, in some restaurants where you take your bill to the register and pay (eg Dennys - don%26#39;t judge) we leave the cash on the table, I hope this is ok to do?
